Making Decisions about Breast Implants: Part 1

Breast augmentation is a highly personalized procedure. Every breast augmentation candidate has a unique idea of what she would like her breasts to look like after surgery. To meet the needs of a diverse group of patients, Dr. Margoles uses implants that differ in size, shape, texture and filler material.

Here are a few ways that implants vary:

Filler Material

Breast implants come filled with either saline or silicone material. Both types of implants are safe and have earned the U.S. Food and Drug Administration’s seal of approval. Saline breast implants have a solid silicone rubber outer shell that is filled with a sterile saltwater solution. Silicone breast implants also have a silicone rubber shell but are filled with a cohesive gel material.

Neither type of filler material is better than the other, but there are advantages to each. If a saline-filled implant ruptures, the solution quickly flushes out of the shell and it becomes immediately apparent that one breast has “deflated.” It is not as easy to detect the rupture of a silicone-filled implant (the FDA recommends women with silicone implants have an MRI every year to check for any problems). On the other hand, many patients find that silicone is lighter and feels more similar to natural breast tissue than saline.


Implant size is a personal decision that the patient makes with the guidance of Dr. Margoles. Some women prefer dramatic results with large implants, and others opt for subtle enhancement with small implants.

Dr. Margoles uses the Mentor Volume Sizing System to help patients determine the best implant size. The Mentor system allows patients to preview what their breasts will look like with different sized implants.


Breast implants are either round or teardrop-shaped. Round implants tend to sit “higher” on the chest and project further from the chest wall. Teardrop implants have a curving slope that is more similar to the shape of natural breasts.


Implants may be either smooth or textured. Textured implants have a rough surface to prevent them from moving around in the implant pocket. Smooth implants move around more easily within the pocket.

Using your Own Fat

Dr. Margoles offers a natural, minimally invasive alternative to traditional breast implants: fat grafting. During breast augmentation with fat grafting, Dr. Margoles extracts fat cells from a “donor area” of your body and re-injects the cells back into the breasts. Depending on several different factors, more than one round of fat grafting may be required to achieve your desired results.
