Who Is The Best Candidate For Eyelid Surgery?

You may be a candidate for eyelid surgery if you have:

  • Droopy upper lids (possibly obscuring the natural fold of the eyelid)
  • Sagging brow
  • Puffiness or “bags” below the eyes
  • Stretched eyelid skin
  • An unintentionally fatigued, angry or sad eye appearance

Candidates for eyelid surgery should be in good overall health, and should not have any health conditions that could raise surgical risks. If you suffer from hypothyroidism, Grave’s disease, glaucoma or dry eye, you are encouraged to consult with your ophthalmologist before pursuing eyelid surgery.

Eyelid surgery will not eliminate dark circles, wrinkles or sagging eyebrows. However Dr. Margoles will review alternative treatment options with you that are better targeted to address these conditions.

Eyelid Surgery Details

Eyelid Surgery (Blepharoplasty) is an out-patient procedure that typically takes 1-2 hours. You will be in the recovery room for 1 hour following surgery, after which you will be sent home. All patients must have a responsible adult with them to return home. It is also strongly advised that a responsible adult stay the night with you.

The surgery is performed by making small incisions where loose skin and pockets of fat have collected. Dr. Margoles will begin by making small incisions along the natural lines of the upper and/or lower eyelids (this prevents visible scarring). Through the incisions, she will remove excess skin and fat and tighten the skin along the lids. She will redistribute fat as needed and correct muscle laxity. The incisions will then be closed with very fine sutures.

Eyelid Surgery Aftercare & Recovery

After the surgery, Dr. Margoles will lubricate your eyes and may advise you to keep your head elevated for several days. Mild side effects such as bruising, swelling, tearing and light sensitivity are normal as the eyes heal. Using a cold compress will help reduce any swelling or bruising. Expected recovery time is between 7-10 days. You will not be able to wear contact lenses during this time. You are encouraged to refrain from any strenuous movement or vigorous activity, including bending, lifting, and exercise for several weeks following your surgery. Dr. Margoles will provide detailed instructions for caring for the incisions, taking antibiotic medication and scheduling your follow-up office visits.

You will receive detailed post-surgery care instructions and Dr. Margoles and her staff will be available to answer any questions or concerns. It is important to follow Dr. Margoles’ instructions carefully to ensure the best results and easiest recovery.

You should see initial results almost immediately after surgery. The results will continue to improve as you heal and post-operative swelling resolves. Your eyelids will look rested, awake and pleasant. Your natural upper eyelid fold will look more defined, and you won’t have puffy under-eye bags. Are the results of blepharoplasty permanent?

Once fat is removed during eyelid surgery, it will never come back. Blepharoplasty does not freeze time, however; you will experience a normal loosening of skin over the next 10 or 20 years. Eyelid surgery has reset the clock, so that natural aging process starts with much younger-looking skin.

Because the result is long-lasting, it is important that a surgeon refrain from removing too much skin. Experienced New York City and Greenwich, Connecticut plastic surgeon Dr. Sandra Margoles favors a conservative approach, excising an amount that is sufficient to give you a relaxed, optimistic look. When too much skin is removed, the resulting side effects may include dry eyes and difficulty completely closing the eyelids.

How do I choose between blepharoplasty and brow lift?

When the main concern is loose skin and an accumulation of fat in the upper and lower eyelids, blepharoplasty is often the best choice. If you are satisfied with the shape and placement of your eyebrow, blepharoplasty may be all you need. Brow lift may be a better choice when your brow is within the eye socket, has lost its natural shape, or you have a pocket of fat or loose skin that involves more than just the eyelid.

What are some of the other alternatives to blepharoplasty?

One alternative to blepharoplasty is to try a dermal filler. Small injections of products such as Bellafill, Restylane, Juvederm or Perlane can improve the appearance of the skin around the eyes by smoothing out the skin. Some patients may also benefit from an advanced technique in which fat from other areas of the body is used to add volume to facial features.

Fillers are an effective way to treat dark circles under the eyes, too. Loss of volume and thinning skin exposes blood vessels under the skin, giving the area a darker appearance. Injecting a dermal filler provides a cushion that compensates for the thinner skin and loss of fat.

Blood vessels and pigment can be directly targeted with laser treatment, as well. Laser is also very effective in treating so-called “crepey” skin under the eyes. In some cases, Botox or Dysport can be a preferred way to relax muscles that are causing wrinkles and volume displacement near the eyes.

Will there be scarring after eyelid surgery?

Scars resulting from eyelid surgery are typically minor. Often they are only visible when you close your eyes. When a surgeon makes the upper eyelid incision, it may sometimes also include a small portion of the “crow’s feet” area. Even here, the resulting mark will be faint. The thin skin of the eyelid is actually less likely to form scars than thicker skin elsewhere on the body.

When fat is removed from the lower eyelid as part of the surgery, incisions are made in the inside of the lower eyelid, where a scar cannot be seen. Sometimes an incision is also made beneath the eyelash; this thin scar is rarely noticeable. Dr. Margoles is experienced in the precise placement of incisions, and will provide post-surgical care that promotes optimal healing to minimize scarring.
