What Is an Eyelid Lift?

An eyelid lift, or blepharoplasty, addresses aging in the upper or lower eyelids by removing excess skin, muscle, and fat to restore a more youthful contour. This procedure can rejuvenate the upper eyelids, the lower eyelids, or both lids simultaneously. To ensure symmetry and balance, Dr. Margoles typically operates on the right and left eyelids during the same surgery, even if sagging primarily affects one side of your face.

An eyelid lift offers patients a host of benefits that include:

  • Eliminate bags under the eyes
  • Soften the appearance of crow’s feet
  • Lift drooping upper eyelids
  • Address dark circles under the eyes
  • Smooth prominent tear troughs

What Is a Brow Lift?

A brow lift addresses aging in the brow and forehead areas of the upper face. This procedure tightens skin and muscles to restore a more youthful facial appearance. The terms “brow lift” and “forehead lift” are interchangeable.

What can a brow lift do for you? The benefits of this procedure include:

  • Restore youthful shape and definition to the eyebrows
  • Look more relaxed
  • Lift drooping brows

Many people pair brow lift surgery with a facelift for a full facial rejuvenation. This approach is popular with male and female patients.

Eyelid Surgery Benefits

Eyelid surgery leaves patients looking more alert, youthful, and rested. In many cases, eyelid surgery may also improve vision by lifting the lid so that it no longer blocks the eye. Initial results are visible almost immediately and will continue to improve throughout the healing process. Along with a more youthful appearance, your natural upper eyelid fold will look more defined, and puffy under-eye bags will be eliminated. The benefits of eyelid surgery will be enjoyed for years to come.

Who Is The Best Candidate For Eyelid Surgery?

You may be a candidate for eyelid surgery if you have:

  • Droopy upper lids (possibly obscuring the natural fold of the eyelid)
  • Sagging brow
  • Puffiness or “bags” below the eyes
  • Stretched eyelid skin
  • An unintentionally fatigued, angry or sad eye appearance

Candidates for eyelid surgery should be in good overall health, and should not have any health conditions that could raise surgical risks. If you suffer from hypothyroidism, Grave’s disease, glaucoma or dry eye, you are encouraged to consult with your ophthalmologist before pursuing eyelid surgery.

Eyelid surgery will not eliminate dark circles, wrinkles or sagging eyebrows. However Dr. Margoles will review alternative treatment options with you that are better targeted to address these conditions.

If you have had LASIK surgery we recommend that you wait for 6 months before scheduling eyelid surgery. When you schedule your consultation, Dr. Margoles will discuss your procedure in detail, including cost, recovery time, follow-up appointments, and medications. She will also be happy to show you before and after pictures to give you a clearer idea of what to expect from your procedure. Dr. Margoles and her staff are always available to answer any questions you may have.

Eyelid surgery performed by Dr. Margoles involves making small incisions where loose skin and pockets of fat have collected. She will begin by making small incisions along the natural lines of the upper and/or lower eyelids (this prevents visible scarring). Through the incisions, she will remove excess skin and fat and tighten the skin along the lids. She will redistribute fat as needed and correct muscle laxity. The incisions will then be closed with very fine sutures.

Minimizing facial scars while achieving natural-looking rejuvenation is critical to Dr. Margoles. Your incision placement will depend on your unique anatomy and aesthetic needs. For an eyelid lift, Dr. Margoles typically places incisions in the upper eyelid crease, inside the eyelid, or along the lash line. These areas hide incisions very well and leave little to no visible scarring. For brow surgery, Dr. Margoles typically makes incisions behind the hairline, where your hair will hide any scarring that may occur.

Greenwich & Westchester, Brow Lift FAQ

When brows sag with age, your expression may seem less joyful, even sad or angry. Lines and wrinkles also begin to form on the forehead and around the eyes. Brow lift surgery is a solution to both issues, restoring an alert, engaged appearance to your expression and smoothing out frown lines, creases and wrinkles. Brow lifts are often done in conjunction with hair transplants, facial fat transfers, eyelid surgery or facelift.

Brow lift performed by an experienced plastic surgeon should leave you looking younger and refreshed, not perpetually surprised. With a brow lift procedure, it is important for the surgeon to pay attention to symmetry in the brow area. If the inner brow is raised more than the outer brow, the look will be unnatural.

The key to attaining the precise elevation and symmetry of the brow is the skill, artistry and judgment of your plastic surgeon. Always look for a surgeon who has performed a procedure many times, and keeps abreast of the latest techniques and medical best practices. An experienced plastic surgeon will take a comprehensive approach; not simply stretching the skin, but also paying attention to the muscles and ligaments under the skin, and how each surgical change affects overall balance and proportion.

Brow lift will improve the appearance of sagging skin, crow’s feet, frown lines and deep lines on the forehead, but not completely eliminate them. With your brow newly elevated and the skin tightened, you will find yourself raising your brows less often, which is what causes forehead lines. If “hooding” of your upper eyelids was caused by loose skin in this area, brow lift will also improve the appearance of skin around the eyes.

Brow lift is not permanent, but you should expect to enjoy your relaxed and refreshed appearance for many years. The procedure is ideal for restoring a youthful look, but it will not eliminate every line or imperfection.

During an endoscopic brow lift procedure, three to five incisions are made beyond your hairline. These half-inch incisions will heal as scar tissue, and hair will not regrow in this type of scar. If your hair is thin, Dr. Margoles can often place incisions farther from the hairline so that these small patches are not noticeable.

Botox®  & Dysport®  are controlled toxins tha tare injected into facial tissue to relax muscles that are forming or sustaining wrinkles. The treatment is quick, convenient and avoids the potential complications of a surgical procedure; however, it must be repeated on a regular basis to keep facial muscles relaxed and prevent lines and furrows from returning.

When Botox is used as an alternative to brow lift, it is injected by the surgeon into the muscles that are pulling down on the brow area. As these muscles relax, they no longer have the strength to continue their tug of war with forehead muscles; this allows the brow to return to a more youthful, natural-looking position for the next three to six months.

Some patients use Botox or dermal fillers to further soften lines even after brow lift surgery, but many find brow lift has given them a youthful, revitalized look that is more natural than that achieved with routine injections.

Greenwich & Westchester, Eyelid Surgery FAQ

Once fat is removed during eyelid surgery, it will never come back. Blepharoplasty surgery does not freeze time, however; you will experience a normal loosening of skin over the next 10 or 20 years. Eyelid surgery has reset the clock, so that natural aging process starts with much younger-looking skin.

Because the result is long-lasting, it is important that a surgeon refrain from removing too much skin. Experienced Greenwich, Connecticut plastic surgeon Dr. Sandra Margoles favors a conservative approach, excising an amount that is sufficient to give you a relaxed, optimistic look. When too much skin is removed, the resulting side effects may include dry eyes and difficulty completely closing the eyelids.

When the main concern is loose skin and an accumulation of fat in the upper and lower eyelids, blepharoplasty is often the best choice. If you are satisfied with the shape and placement of your eyebrow, blepharoplasty may be all you need. Brow lift may be a better choice when your brow is within the eye socket, has lost its natural shape, or you have a pocket of fat or loose skin that involves more than just the eyelid.

One alternative to blepharoplasty is to try a dermal filler. Small injections of products such as Bellafill® , Restylane® , Juvederm®  or Perlane®  can improve the appearance of the skin around the eyes by smoothing out the skin. Some patients may also benefit from an advanced technique in which fat from other areas of the body is used to add volume to facial features.

Fillers are an effective way to treat dark circles under the eyes, too. Loss of volume and thinning skin exposes blood vessels under the skin, giving the area a darker appearance. Injecting a dermal filler provides a cushion that compensates for the thinner skin and loss of fat.

Blood vessels and pigment can be directly targeted with laser treatment, as well. Laser is also very effective in treating so-called “crepey” skin under the eyes. In some cases, Botox® or Dysport®  can be a preferred way to relax muscles that are causing wrinkles and volume displacement near the eyes.

Scars resulting from eyelid surgery are typically minor. Often they are only visible when you close your eyes. When a surgeon makes the upper eyelid incision, it may sometimes also include a small portion of the “crow’s feet” area. Even here, the resulting mark will be faint. The thin skin of the eyelid is actually less likely to form scars than thicker skin elsewhere on the body.

When fat is removed from the lower eyelid as part of lower blepharoplasty, incisions are made in the inside of the lower eyelid, where a scar cannot be seen. Sometimes an incision is also made beneath the eyelash; this thin scar is rarely noticeable. Dr. Margoles is experienced in the precise placement of incisions, and will provide post-surgical care that promotes optimal healing to minimize scarring.
